영화는 전편이 끝난 바로 몇 초 후의 상황을 보여주며 시작된다. 취재진과 소방관들이 모두 감염되고 남은 사람은 리포터 안젤라 뿐. 좀비 바이러스에 완전히 잠식당한 아파트는 봉쇄되고, 그 안으로 상황을 파악할 SWAT팀과 목적을 알 수 없는 오웬 박사가 투입된다.

네명의 기록영화팀이 아마존 오지의 비밀을 필름에 담기 위해 출발하지만, 두달이 지나도록 소식이 없다. 그들을 후원했던 방송국에서는 구조대를 특파하고 아마존에 도착한 구조대는 기록영화팀의 참혹한 죽음의 소식과 함께 그들이 남긴 필름을 가지고 돌아온다. 필름을 현상한 방송국에서는 이 필름을 방영하면 높은 시청률을 이끌어 낼 수 있을 것이라 생각하고 시사를 하지만 그 내용의 참혹함에 놀라 결국 필름을 소각시켜 버린다. '지옥으로 가는 길'이란 제목이 붙어있는 그 필름에는 기록영화팀이 원시의 생활 속에서 차츰 잔인한 본능을 드러내고 평화롭게 살고 있던 원주민들을 잔인하게 살해하는 모습이 담겨있었는데...

공포 영화를 찍으러 인적 드문 야외로 나간 대학생들이 우연히 좀비를 만나게 되고, 이후 촬영은 예측불가능한 방향으로 흘러간다. 친구, 애인 혹은 가족까지 언제 좀비가 되어 덮칠 지 모르는 상황에서 주인공들은 현장을 전 세계에 중계하겠다는 일념으로 카메라를 손에서 놓지 못한다. 흥행과 관계 없이 속편이 계획되어 있는 작품으로 극도의 잔인함 속에서 인간성에 대한 새로운 고찰을 보여준다.

한밤중에 일하는 사람들을 인터뷰하는 리얼 TV 프로그램의 리포터 안젤라와 카메라맨 스캇은 촬영을 위해 LA의 911구조대를 방문한다. 구조대원들의 일상을 취재하던 중, 한통의 구조요청 전화가 걸려오고, 소방대원들을 따라 현장인 다운타운의 작은 아파트에 도착한 안젤라와 스캇은 사건현장을 최대한 밀착취재하고자 한다. 그곳에서 일행이 발견한 것은 피로 범벅이된채 어둠속에 서 있는 노파. 구조대원이 그녀를 도우려 다가간 순간, 갑자기 노파가 대원을 공격하는 일이 발생한다. 심상치 않은 일들이 일어나고 있음을 깨달은 일행들은 노파와 부상당한 구조대원을 데리고 건물을 빠져나가려 하지만, 당국의 폐쇄조치로 출입문은 이미 잠긴 상태이다. 건물안의 사람들은 하나둘 좀비로 변하고, 변하지 않은 사람들은 살기위해 필사적으로 도주한다. 안젤라와 스캇의 카메라는 쉴틈없이 이 모든 상황들을 찍어나가는데...
