The story takes place within Neo Tokyo in the year 2025, five years after a severe terrorist attack at the 2020 Tokyo Olympics. Students possessing special abilities are forced into serving an elite anti-terrorist unit created by the United Nations, fighting against threats including other ability users. If these agents fail their mission, they will be killed along with their families, who have bombs planted within their heads. One member of this covert ops unit, a Swedish girl by the name of Anthea Kallenberg, seeks to find who she really is.

Summer, 2006. A 5th grader Ryuhei discovers a notebook entitled "Tokyo Exploring Records." It was a journal kept by his father, Shohei, when he was around Ryuhei's age. Ryuhei forms his own exploration party with his friends Shun and Yoshio. A film adaptation of the Holy Emperor story arc, which primarily depicted the conflict between Kenshiro and Souther. New characters Reina, one of Raoh's army officers who falls in love with him, and her brother Soga, Raoh's advisor, play an important part with much of the plot involving Raoh's relationship with Reina as he conquers the land; most of this portion is new content exclusive to this film. The other side of the story is the retelling of Ken's attempt to save and protect the villagers from Souther's army with the help of Shū. There is also a small subplot of Bart returning to his home.

In Japan, the vampire-hunter Saya, who is a powerful original, is sent by her liaison with the government, David, posed as a teenage student to the Yokota High School on the eve of Halloween to hunt down vampires. Saya asks David to give a new katana to her. Soon she saves the school nurse Makiho Amano from two vampires disguised of classmates and Makiho witnesses her fight against the powerful demon.
