During this time, Black Jack receives an urgent request from a past patient whose identity shocks him: Yurie SAIONJI, whose Teratogenous Cystoma was constructed into Pinoko by Black Jack 10 years ago. In Part 2, Black Jack is kidnapped by the government of a war-torn country.

The film involves the team-up of five different Super Sentai teams (Ninja Sentai Kakuranger, Gosei Sentai Dairanger, Kyōryū Sentai Zyuranger, Chōjin Sentai Jetman, and Chikyū Sentai Fiveman), who must defend the planet Earth from the evil demon Emperor Daidas. In addition to the five Sentai teams, the film also features Shiro Izumi, who played Yūma Ōzora in Dengeki Sentai Changeman and Burai in Zyuranger, as Masato, a young man who witnesses the Emperor's weak point with his sister Ayumi. Masato and Ayumi also appear in Kamen Rider World, linking the two movies. The narration was provided by Hironori Miyata, the narrator from Dairanger.

The story begins with three Aliens in human form preparing for a ritual to feed the Fog Mother's "siblings". The Fog Mother last came to earth during the Prehistoric Age and is the cause of the extinction of the dinosaurs. Kenji and his kid sister are in the mountains, investigating all the deaths of animals and trees there. The Fog Mother sends the three aliens, "Gorai", "Zuu", and "Agito" to kidnap Kenji's baby sister for the sacrifice.

He fled into the mountainside and went into a coma before he was awakened by a telepathic call two years later with an unconscious urge to protect Hiroshi Mochizuki, the son of Doctor Mochizuki.

The city of Yūbari in the Hokkaido prefecture has become a ghost town following the presence of Shadow Moon and his minions of Gorgom. Makino, a scientist working on Gorgom's top-secret robot experiment, escapes from the evil organization and returns to Tokyo, only to find out that his wife and daughter have been abducted. As Kamen Rider Black, Kotaro travels to Yubari to save Makino's family and liberate the town from Shadow Moon's evil grasp.

After the fall of the Tokugawa Shogunate, there was a series of battles fought while the former supporters of the Tokugawa shogunate retreated to the north where they actually started a sovereign nation that was recognized by more than one European country. Survivors of the Shinsengumi were among the followers of Enomoto Takeaki who took them to the northernmost island of Ezo where they fought their final battle at the star shaped fort, Goryokaku. The Japanese Civil Wars fought in the name of the emperor signaled the complete end of the feudal system and Japan’s entry into the modern world as those brave samurai tried to halt progress and learned that the age of modern warfare and weaponry had passed them by. Swords were no match for rifles and cannons, nor was any man a match for the power of the imperial flag. Japanese loyalty to the emperor has long defined the nation and culture despite the changing times.
