La película gira en torno al regreso de Xander Cage, a quien todos creían muerto. Cage, alias xXx (Triple X), es un rebelde amante de los deportes extremos que adora romper las leyes y que por ello acaba siendo atrapado por las autoridades de EE.UU. Entonces se ve obligado a colaborar como agente encubierto participando en numerosos episodios de riesgo. Tras un fatídico incidente, Xander es dado por muerto. Pero ahora, vuelve a la acción de incógnita acompañado de su agente instructor Augustus Gibbons.
