Don Salvatore Savastano, a boss involved in the smuggling of contraband cigarettes, is forced to watch four masked robbers kill his wife and his only son in a restaurant where he and his family are celebrating the young boy's first Holy Communion. From that moment on Don Salvatore has no other purpose in life but to get his revenge on the four assassins.

La clínica psiquiátrica, dirigida por el Dr. Oesterman, es un lugar de residencia para jóvenes con distintas turbaciones mentales, todas provenientes de familias de alcurnia, tales como una ninfómana, una lesbiana, una paciente, enamorada del Dr. Keller, etc. Pero la tranquilidad se ve alterada con el asesinato de una enfermera, seguido de una agresión a la última paciente nombrada; el dueño del lugar, en pos de cuidar el buen nombre del sanatorio, decide no dar parte a la policía, dando pie al asesino para que continúe sus andanzas.
