The final candidates for a highly desirable corporate job are locked together in an exam room and given a test so simple and confusing that tension begins to unravel.

A deadly virus has spread across the globe. Contagion is everywhere, no one is safe, and no one can be trusted. Four friends race through the back roads of the American West on their way to a secluded utopian beach in the Gulf of Mexico where they could peacefully wait out the pandemic. Their plans take a grim turn when their car breaks down on an isolated road starting a chain of events that will seal their fates.

症状がレベル5まで進行した感染者は、凶暴化して人間を襲い始めるという危険なウイルスが蔓延するアメリカ。ある秘密と目的を抱えながら、CDC（疾病管理予防センター）のドクターとして、感染者から未感染者を守る救助チームへの配属を希望した女性ローレン。彼女の狙いは、本来感染者は救助不能とされるところ、ドクターなど貴重な人材の場合に限り、レベル1までなら治療できる特効薬を使えるという特権にあった。

アメリカ合衆国中西部に位置するラクーンシティ。自然豊かなこの街の郊外に以前は製薬会社アンブレラ社の工場が存在したが、今はその殆どの施設は移転してしまっている。 このアンブレラ社が秘密裏に研究開発を進めていた“何か”が街の住民達に大きな健康被害を与えているとのメッセージを受け取ったクレア。 ラクーンシティの施設で育ったクレアは、その真実を突き止めるべく、R.P.D．(ラクーン市警)で特殊部隊＝S.T.A.R.S.の隊員である兄・クリスのもとを訪ねる。クレアはクリスにこの事実を訴えるも「お前は昔から陰謀論を持ち出すが、デタラメだ」と取り合ってくれない。 しかしその時、街中に大音量のサイレンが鳴り響く。 アンブレラ社から住民に自宅で待機するよう警報が発せられたのだ。 クリスは急ぎ署に出向き、S.T.A.R.S.の隊員である、ジル、ウェスカーと共に、郊外にあるスペンサー邸で消息を絶った同僚を捜索する為、ヘリコプターで出動する－。 一方、クレアはクリスを追いかけR.P.D．に－。 しかし、既に住民達の身体には変化が起き始めていた。 その皮膚は腐乱し、口や目から血液が流れ落ち、死体の様な状態にも関わらず、人肉を欲し彷徨うゾンビと化したのだ。 スペンサー邸ではクリス達の壮絶なサバイバルが繰り広げられ、R.P.D．内でも、クレアそして新人警官のレオンに、ゾンビ達が襲い掛かる。

夜やってくる“それ”の感染から逃れるため、森の奥でひっそりと暮らすポール一家。そこにウィルと名乗る男とその家族が助けを求めてやって来る。ポールは“それ”の侵入を防ぐため「夜入口の赤いドアは常にロックする」というこの家のルールに従うことを条件に彼らを受け入れる。うまく回り始めたかに思えた共同生活だったが、ある夜、赤いドアが開いていたことが発覚。誰かが感染したことを疑うも、今度はポール一家の犬が何者かによる外傷を負って発見され、さらにはある人物の不可解な発言…外から迫る、姿が見えない外部の恐怖に耐え続け、家の中には相互不信と狂気が渦巻く。彼らを追い詰める“それ”とは一体・・・。
