ローズとクリストファーは、娘・シャロンの奇妙な言動に悩んでいた。ローズは、しばしば情緒不安定に陥り、何かに取り憑かれたかのように「サイレントヒル…」と謎のうめき声を発する娘に心を痛め、サイレントヒルという街が実在することを知り、治療のためにシャロンを連れてそこを訪ねることにする。しかし、サイレントヒルは30年前の大火によって多数の人々が死亡した忌まわしい場所であり、今では誰も近付かない廃墟と化した街であった。途中、女児誘拐犯と勘違いされて女性警官に追われるものの、二人とも帰り道が突如切れ無くなった街に取り残され、シャロンも後部座席から消えており、捜索、他の住人への聞き取りなど進めていくが不思議な現象に巻き込まれてゆく。

ある日、ハーバードヴィル空港へ降り立ったクレアは、利用客達の中に1人のゾンビを発見した。そのゾンビに噛まれ次々と感染者が増大し空港は大パニックとなる。一方、空港へ向かって飛行中の旅客機内でも感染者が発生し、緊急着陸を試みるも失敗して空港ロビーへ激突してしまう。まもなく、壊れた機体からは乗員や乗客が次々と姿を現すが、彼らは皆ゾンビ化していた。この事態によってクレアは、居合わせたデイビスや知人の姪のラーニー達と共に、空港の一室へ立てこもることとなる。

T-ウイルスは世界中に蔓延し、遂に東京でも感染者が出た。そして、世界滅亡のカウントダウンが始まった。それから4年後。アンブレラ社は東京、渋谷の地下に巨大要塞を築き、アルバート･ウェスカー議長の指示の下で実験を繰り返していた。その地下要塞を、アリスが自身のクローンたちと共に襲撃。地下要塞は壊滅的なダメージを受ける。 ウェスカーは地下要塞をあっさり見限り飛行機で一人逃亡、予め仕掛けていた特殊爆弾[4]を爆破する。クローンアリスもろとも東京は地下要塞ごと壊滅した。 ウェスカーは勝利を確信するが、オリジナルのアリスは生き延びてウェスカーと同じ飛行機の中にいた。アリスはウェスカーを追い詰めるが、不意打ちを受けて取り逃がしてしまう。

21世紀初頭。全米No.1の巨大複合企業であり、アメリカでの家庭用医薬品シェア90%を誇るアンブレラ社。その裏の姿は、細菌兵器の開発などを行う「死の商人」であった。ある日、ラクーンシティにある秘密地下研究所で、研究中のT-ウイルスが何者かの手によって漏れ出し空調設備を通じて所員全員がT-ウイルスに感染した。ウイルスの漏えいを防ぐため施設の制御コンピュータ「レッド・クイーン」はハイブ内の各通路を封鎖し、500人を超える所員全員を死亡させて汚染を施設内に封じ込めた。この事故を知ったアンブレラ社は、事故の原因はレッド・クイーンの誤作動と判断しレッド・クイーンをシャットダウンするため特殊部隊を現地へ派遣する。
