ミッチの妻・ベスは、香港への出張後にシカゴで元恋人と密会していたが、せきと熱の症状が出始める。同じころ香港、ロンドン、東京で似たような症状で亡くなる人が続出。フリージャーナリストのアランは伝染病ではないかと考え始める。

A deadly virus has spread across the globe. Contagion is everywhere, no one is safe, and no one can be trusted. Four friends race through the back roads of the American West on their way to a secluded utopian beach in the Gulf of Mexico where they could peacefully wait out the pandemic. Their plans take a grim turn when their car breaks down on an isolated road starting a chain of events that will seal their fates.

症状がレベル5まで進行した感染者は、凶暴化して人間を襲い始めるという危険なウイルスが蔓延するアメリカ。ある秘密と目的を抱えながら、CDC（疾病管理予防センター）のドクターとして、感染者から未感染者を守る救助チームへの配属を希望した女性ローレン。彼女の狙いは、本来感染者は救助不能とされるところ、ドクターなど貴重な人材の場合に限り、レベル1までなら治療できる特効薬を使えるという特権にあった。

多くの犠牲を払いながら巨大迷路を脱出したトーマスたちは、同じように別の迷路を生き延びた若者たちが収容された施設へと運ばれた。つかの間の休息を過ごすトーマスたちだが、やがて彼らはこの施設の恐るべき実態を知り、彼らを迷路に送り込んだ黒幕である組織WCKD（ウィケッド）の存在に気付く。危険を察知したトーマスたちは仲間とともに脱出、だが施設の外には、組織が“第2ステージ”と呼ぶ砂漠の迷宮が広がっていた。
