DOUGLAS SIKES, a convicted war criminal from the American Zombie War, runs a black-market trading ring from the bowels of a walled-in quarantine zone turned for-profit prison. He and the other prisoners live under the watchful eye of the ZTV broadcasting company, and at the whim of THE PREZERVE, a capital punishment reality game show that uses zombies, and horrific devices, to torture, infect, and execute prisoners and their families on prime time network television. PAUL, and his daughter, LILY, arrive at Doug's home to find him grief-stricken and consumed with rage after his daughter Steffie's gruesome episode has broadcast her terrible fate for all the world to see. Paul and Lily find themselves in the cross-fire as Doug readies to unleash bloody vengeance on the institution and the vengeful woman who has oppressed him.

ゾンビ映画の元祖「ナイト・オブ・ザ・リビング・デッド／ゾンビの誕生」で描かれた事が実際にあったという前提で作られた間接的な続編。ロスにある科学資料庫の地下で発見された謎のタンク。そこから吹き出した特殊なガスには死者を蘇らせる作用があった……。
ある日突然死んだはずの人間が次々とゾンビになり、そのうちの一人が知恵をつけ始めた。銃や、器具を使うことを覚えたゾンビたちにライリー（サイモン・ベイカー）やわずかな生存者たちは窮地に追い込まれる。
動物愛護活動家が医療科学研究所を襲撃した。研究員はチンパンジーが危険な感染症にかかっていると警告するが活動家は耳を貸そうとしない。檻から出した途端、チンパンジーは研究員や活動家に襲いかかった。28日後。交通事故で昏睡状態に陥っていたバイク・メッセンジャーのジムは、ロンドン市内の病院の集中治療室で意識を取り戻す。ベッドから起き廊下をさまようジムだったが院内にはまったく人の気配がなかった。人の影を求めて街へ飛び出したジムは、そこで驚くべき光景を目にする。
ウイルスの発生から28週後、安全宣言が出されたロンドンでは米軍を主導としたNATO軍の保護下で都市の復興が進んでいた。そして保護地区で暮らし始めたドンのもとにスペインに旅行していた子供達、タミーとアンディが帰ってくる。
21世紀初頭。全米No.1の巨大複合企業であり、アメリカでの家庭用医薬品シェア90%を誇るアンブレラ社。その裏の姿は、細菌兵器の開発などを行う「死の商人」であった。ある日、ラクーンシティにある秘密地下研究所で、研究中のT-ウイルスが何者かの手によって漏れ出し空調設備を通じて所員全員がT-ウイルスに感染した。ウイルスの漏えいを防ぐため施設の制御コンピュータ「レッド・クイーン」はハイブ内の各通路を封鎖し、500人を超える所員全員を死亡させて汚染を施設内に封じ込めた。この事故を知ったアンブレラ社は、事故の原因はレッド・クイーンの誤作動と判断しレッド・クイーンをシャットダウンするため特殊部隊を現地へ派遣する。
アンブレラ社の地下秘密研究所「ハイブ」でのバイオハザード発生から2日後、その真上にあるラクーンシティでは住民達がいつもと変わらぬ日常を送っていた。しかし、ハイブで起きた事件を調査すべく派遣されたアンブレラ社の社員が封鎖された地下入り口を開けると、高濃度のウイルス反応が検知され調査隊は内部から出てきたクリーチャーによって襲われてしまう。その結果、ウイルスによる汚染が街にまで蔓延し人々は次々と感染してゾンビ化、街中で人を襲い始めてアンデッド達が溢れ出し、壊滅状態になった。事件の隠蔽を図るアンブレラ社はラクーンシティを封鎖する。

トランシルバニア。ドラキュラが支配人を務める“モンスター・ホテル”は今日も大盛況。ドラキュラの娘メイヴィスと人間の青年ジョニーとの間にはかわいい息子デニスも誕生した。そんな矢先、息子を普通の人間として育てたいメイヴィスたちと立派なモンスターにしたいドラキュラが衝突、メイヴィスたちは米国に引っ越すべく家探しを始める。一方、ドラキュラは彼らの留守中、デニスにモンスターの英才教育を施そうとする。
