州立精神病院にひとりの男が収容された。彼の名はR·P·マクマーフィ。極めて自由な精神を持ち、体制やルールに徹底して反発する彼は、慇懃な仮面を被った絶対権力者であるラチェット看護士と衝突する。不屈の姿勢を崩さないマクマーフィの影響で、体制によって無気力な人間にされていた患者たちが次第に心を取り戻し始めるのだが…。
