世界唯一の脱獄のプロフェッショナル、ブレスリン(シルヴェスター・スタローン)。彼が大海原に浮かぶ監獄要塞“墓場”を攻略して数年が経過し、現在は一流のスタッフを集めた警備会社を新設、第一線からは距離を置いていた。そんなある日、従兄弟のユシェンの警備にあたっていたスタッフの一人シューが姿を消す。足取りを追うブレスリンであったが、全容が謎に包まれた“ハデス”と呼ばれる監獄にシューが勾留されているとの噂を耳にする。そこは全てがコンピューター制御された“墓場”を超える最強セキュリティの監獄だ。“ハデス”の謎を追って一人、また一人と消えていくスタッフたち。これはブレスリンを誘い込む罠なのか！？ブレスリンはかつての友人デローサ(デイヴ・バウティスタ)の協力を仰ぎ、難攻不落の監獄へ再び挑むのだが…。
