暗殺の仕事から離れ、平穏な日々を送るビショップのもとに、かつてビショップを裏切り姿を消したままでいた兄弟子クレインから暗殺の依頼が入る。相手にしないつもりだったが、人質を取られて引き受けざるを得ない事態になる。フィクサーとして世界を裏で操る3人の武器商人がターゲットとして提示され、事故死に見せかける暗殺計画を進めるビショップ。しかし、クレインが計画遂行後に自分を殺そうとたくらんでいるのを知る。

世界ヘビー級チャンピオンだった父親、アポロ・クリードを知らずに育ったアドニス・ジョンソン。父親を生前に亡くしたが、その体には確実にボクサーの血が受け継がれていた。アドニスはフィラデルフィアに向かい、アポロ・クリードと新鋭ロッキー・バルボアの伝説の試合が行われた場所へ足を運ぶ。「兄弟愛の街」という意味を持つフィラデルフィアで、アドニスはロッキーを探し出し、トレーナーになってくれるよう頼む。ロッキーは二度とボクシングに関わらないと心に決めていたが、アドニスの姿に、かつて宿敵で親友だったアポロの強靱さと闘志を見出す。ロッキーは若きファイター、アドニスの頼みを聞き入れ指導を始める。元チャンピオンのロッキーはリングで戦ってきたどの相手よりも真剣にアドニスと対峙する。ロッキーをセコンドにつけたアドニスは、チャンピオンに手が届くところまですぐに昇り詰める。アドニスは果たしてリングに上がるまでに、闘志だけでなく真のファイターとしてのハートを手に入れられるのか？
