When the champ's promoter, Rev. Sultan, decides something new is needed to boost the marketability of the boxing matches, he searches and finds the only man to ever beat the champ. The problem is that he isn't a boxer anymore and he's white. However, once Rev. アポロとの死闘で判定負けしたロッキーは、危険なボクシングから足を洗い、恋人エイドリアンと結婚。正業に就いて家庭を築こうとするが、ボクシング一筋に生きてきた彼はどんな仕事をしてもうまくいかない。一方、無名のロッキーをKOできなかったアポロはチャンピオンとしてのプライドを打ち砕かれ、評価も人気も失っていた。復讐に燃えるアポロはマスコミを使ってロッキーを挑発、再戦のリングに引きずり上げようと画策する。

フィラデルフィアの下町。無名ボクサーのロッキーは本業だけでは食えず、借金の取りたてを請け負って日銭を稼いでいた。そんなある日、世界チャンピオンのアポロが気まぐれで無名選手にチャンスを与えようと言い出し、無作為に選んだロッキーを挑戦者に指名する。降って湧いたチャンスを得て、ロッキーは想いを寄せる女性エイドリアンに、15ラウンド最後まで戦いぬくことで自分の愛を証明すると約束する。
