ルーマニアのカルパチア地方からイギリスのロンドンまで、謎めいた50個の無記名の木箱を運ぶためチャーターされた帆船デメテル号は、その航海の途上で毎夜、不可解な出来事に遭遇する。デメテル号船長の航海日誌に記されたおよそ1カ月におよぶ無慈悲な存在との対峙の記録をもとに、大海原をわたるデメテル号に何が起こったのか、そして謎に包まれた50個の木箱の中身をめぐる恐怖の物語が展開する。

前作での出来事の後、ゾーイとベンはエスケープ・ルームを仕掛けた企業、ミノス社を打倒すべく行動を開始した。ところが、2人はその過程で再びトラップだらけの部屋に閉じ込められてしまう。不可解なことに、その部屋にはかつてエスケープ・ルームから生還した人々（レイチェル、ブリアナ、ネイサン、テオ）が勢ぞろいしていた。ミノスの目的が分からないまま、ゾーイたちは生き残るために必死で知恵を絞っていくが、その果てにたどり着いたのは何とも悲しく、恐ろしい真実であった。

とある高層ビルの一室に集まった6人の男女。彼らは謎の主催者から賞金1万ドルという脱出ゲームに招待された参加者だった。リラックスしてゲーム開始を待つ6人だが、一向に主催者からの連絡がない。待ちくたびれた彼らが帰ろうとすると、ドアノブが外れ、何の説明もなくゲームが開始される。出口は開かず、次第に部屋の温度が上昇していく。やがて6人は、これが遊びではなく、失敗すれば命がないデスゲームであることを知る。

カプコンの人気ゲームを映画化したミラ・ジョボビッチ主演による人気アクション「バイオハザード」シリーズの最終作。人類の大半がアンデッドと化した世界で、人類最後の希望となったアリスは、悪夢のような現実の全ての始まりの場所、ラクーンシティのハイブへ戻ることになる。しかし、そこでは全ての元凶である巨大企業アンブレラ社が、アリスとの最終決戦に向けて全勢力を結集させていた。ジョボビッチ扮するヒロインのアリスほか、ゲーム版の人気キャラクターでもあるクレア・レッドフィールドを演じたアリ・ラーターが、4作目以来に同役でカムバック。また、日本の人気タレントのローラが、アリスと共闘する女戦士コバルト役でハリウッドデビューを果たした。監督はジョボビッチの夫で、シリーズ3、4作目をのぞいてメガホンをとってきたポール・W・S・アンダーソン。
