アンブレラ社の地下秘密研究所「ハイブ」でのバイオハザード発生から2日後、その真上にあるラクーンシティでは住民達がいつもと変わらぬ日常を送っていた。しかし、ハイブで起きた事件を調査すべく派遣されたアンブレラ社の社員が封鎖された地下入り口を開けると、高濃度のウイルス反応が検知され調査隊は内部から出てきたクリーチャーによって襲われてしまう。その結果、ウイルスによる汚染が街にまで蔓延し人々は次々と感染してゾンビ化、街中で人を襲い始めてアンデッド達が溢れ出し、壊滅状態になった。事件の隠蔽を図るアンブレラ社はラクーンシティを封鎖する。

密輸品取引現場に踏み込んだFBIにより、裏社会の大物セイントの息子ボビーが射殺される。憤怒したセイントは裏切り者を捜さんと、潜入捜査をしていたFBIのフランクを見つけ出す。家族を守るために転任を決意し、束の間の休暇を楽しんでいたフランクだが、そこへセイントの追っ手が現われ、妻子を殺されてしまう。ただひとり生き残ったフランクだが絶望の中から立ち上がり、闇の私刑執行人“パニッシャー”へと生まれ変わる。
