영화는 왓슨의 사후 50년 후 은행 금고에 맡겨둔 홈즈의 유품을 공개하는 것으로 시작된다. 왓슨은 과학 추리를 예술의 경지로 끌어올린 자신의 가장 절친한 친구이자 최고의 명탐정인 홈즈의 사건 중에서 자칫 홈즈의 스캔들이 될 수도 있는 사건의 내용을 공개하지 않고 홈즈의 유품과 함께 보관해 두었다. 때는 1887년 8월. 애버네티 장군의 살인 사건을 해결하고 돌아온 홈즈가 베이커 가의 하숙집에서 무료한 나날을 보내던 중, 갑자기 한밤중에 벨기에 출신의 발라동 부인이 홈즈의 집에 나타나고, 발라동 부인은 공기펌프 기술자인 남편이 영국의 요나 회사에 취직이 된 후 행방불명 됐다며 남편인 에밀 발라동을 찾아달라고 홈즈에게 부탁한다. 발라동 부인의 묘한 매력에 끌리던 홈즈는 사건 수사를 시작하지만, 몇 가지 단서를 찾을 무렵 갑자기 형인 마이크로프트가 나타나 영국 여왕과 관련된 사건이라며 홈즈에게 당장 사건을 정리하라고 경고한다. 그러나 홈즈는 형의 경고에도 불구하고 형의 실험실에서 알아낸 몇 가지 단서를 가지고 발라동 부인과 애쉬돈 부부 행세를 하며 스코틀랜드의 인버네스로 향한다. 홈즈의 일행은 공동묘지에서 찾은 에밀 발라동의 시신에서 에밀이 질식사했음을 확인하고, 네스 호의 괴물이 공기펌프가 달린 중요한 장치임을 밝혀낸다. 그러나 이때 또다시 등장한 형 마이크로프트에게 사건의 전말을 듣게 되는데... 사건의 전말은 이렇다. 홈즈의 형인 마이크로프트는 에밀이 발명한 공기펌프를 이용해서 잠수함 개발에 박차를 가하고, 독일은 공기펌프 기술을 빼내기 위해 발라동 부인으로 가장한 미모의 첩보원을 홈즈에게 보낸 것이다. 결국 네스 호의 괴물은 마이크로프트가 만든 잠수함 요나 호였으며, 에밀 발라동은 실험 도중에 사망한 것이었다.
