왓츠와 그의 팀은 귀신이 나온다는 악명 높은 폐쇄 교도소에 가서 생존자를 확인하고 건물을 폭파하라는 명령을 받고 출동한다. 기술 담당자인 후퍼가 개발한 특수 카메라가 달린 안경을 쓰고 교도소 안으로 침투한다. 그 안에서 1992년에 실종된 엠마를 발견하게 된다. 그녀는 616이라는 숫자와 관련된 모든 사건과 연관되어 있으며 아직까지 풀지 못한 미제사건의 용의자 이다. 초심리학자인 테커 박사는 그녀를 감방에 가두어 놓는다. 그 이후 교도소 안을 조사하던 대원들은 알 수 없는 공포의 환상에 빠져 차례대로 죽어나가고.. 이런 상황을 더는 지켜볼 수 없던 왓츠는 바로 건물에 폭탄을 설치하려고 한다. 하지만 감방에 있던 엠마로 인해 또 다른 충격적인 살인 사건들이 발생하는데...
