정신과 의사로 일하는 프레드는 가족은 둘째고 일만 하기 급급한 가장이다. 어느 날 주지사 후보자인 필브룩에게 어떤 문서에 서명하라는 부탁을 받는다. 자세한 정황도 알지 못한 채 가족을 부자로 만들기 위해 필브룩이 시키는 대로 서명하기 위해 프리츠패트릭 정신병원에 찾아간 프레드. 한편 프리츠패트릭 정신병원에는 프레드와 어릴 때 헤어진 쌍둥이 동생 테디가 환자로 있었다. 자기도 가족을 갖고 싶다는 생각에 테디는 우연히 병원에 찾아온 자신의 쌍둥이 프레드와 신분을 바꿔치기하고 프레드 행세를 하기 시작한다. 영문도 모른채 정신병 환자 취급을 받으며 정신병원에 갇힌 프레드. 아무도 자기가 의사임을 믿어주지 않자 결국 탈출 계획을 세운다. 그러던 중 자신이 서명한 문서가 돌로레스라는 평범한 아이를 정신 병원에 가두기 위한 문서였음을 알게 되고, 한편 프레드 행세를 하던 테디는 꿈에 그리던 가족을 행복하게 해주기 위해 노력하며 살아간다. 운명이 갈린 쌍둥이 형제가 정신 병원에 얽힌 에피소드를 그리며 가족애를 그린 코미디 영화
