콜은 요양원에서 일하는 젊고 건강한 요양보호사로 애팔래치아 시골 마을에 살고 있다. 도시의 남는 의약품을 지역 주민들에게 판매하며 근근이 살아가던 콜. 이 마을에는 돈 벌 기회가 별로 없지만, 약물이 풍부하다는 것을 알게 되면서 마약 거래상의 길을 걷는다. 공급책들을 진심으로 아끼고 마약 중독자들을 위험한 약물로부터 보호하는 등 이중생활을 하던 콜의 위태로운 삶은 몇 년 만에 귀향한 어린 시절 친구 테리 로즈가 그의 자리를 노리기 시작하며 흔들리기 시작한다.
