Todo ano é realizado na remota e exótica ilha Doatec o torneio Dead or Alive, organizado pelo dr. Victor Donovan. O torneio reúne os maiores lutadores do mundo, incluindo os melhores em cada estilo de luta. Entre os concorrentes deste ano está Tina Armstrong, a campeã mundial de luta livre, a ladra Christie Allen e Katsumi, uma ninja que tem sede de vingança devido à morte de seu irmão.

A história centra-se em um jovem de Birmingham (Alabama), interpretado por Jeff Bridges, que está envolvido em um acordo desonesto.
