Isabella Rossi é uma jovem americana que viveu toda a vida marcada pela tragédia envolvendo a sua mãe, Maria Rossi, que assassinou brutalmente três padres católicos numa tentativa fracassada de exorcismo, em 1989. Internada na ala psiquiátrica de um hospital em Roma, ao abrigo dos cuidados do Vaticano, Maria nunca recuperou e é considerada uma doente violenta e perigosa. Mas Isabella crê que a mãe está verdadeiramente possuída e vai para Roma na esperança de descobrir a verdade e convencer a Igreja a autorizar uma nova tentativa de exorcismo.
