A história centra-se na Guerra do Santo Graal e explora a relação entre Emiya Shirou e Matou Sakura, dois adolescentes que participaram neste conflito. A história continua imediatamente depois de Fate / stay night: Heaven's Feel I. Presage Flower, seguindo Shirou como ele continua a participar na Guerra do Santo Graal, mesmo depois de ser eliminado como um mestre.

A história segue Matou Sakura, a irmã mais nova de Tohsaka Rin, que quando jovem foi adotada pela família Matou. Assim, ela teve que passar por um torturante treinamento para desenvolver seus poderes mágicos. Sakura não deveria se envolver na Guerra do Santo Graal, mas devido ao seu envolvimento romântico com Shirou, sua mente quebra e ela fica louca. Sakura se torna uma líder da Guerra do Santo Graal e força Shirou em um Destino Cruel.

Mikiya Kokutou gives Shiki Ryougi a cat to watch, as he will be away for a little while. Though Shiki protests, he leaves the cat anyway, and Shiki is stuck trying to understand her new feline companion. But as luck would have it, the cat seems to have taken a liking to Mikiya and misses him. Later in another part of the city, Ririsu Miyazuki visits the place where her dear friend committed suicide. She intends to end her life as well, but she meets Fujino Asagami, a blind classmate with a traumatic past. Though the two girls don't have much in common, Asagami reminds Miyazuki of her lost friend and helps her understand her pain. Two months later, Mikiya elects to celebrate New Year's with Shiki instead of his family, which makes his sister Azaka very upset, leading to her spending the holiday with her school friends. As the snow begins to fall, Mikiya reflects on what he wishes for most of all: that Shiki's life be filled with happiness.

Na história composta por dois contos e três capítulos do mangá, o filme conta a história de dois físicos, Seo Shizune e Kuramitsu Meruka, que podem ver o futuro. Vamos ver o que ocorreu dez anos após o último Kara no Kyoukai, e sobre o dia que Mana Ryougi, filha de Shiki, passou com Mitsuru Kamekura.
