不運な事件に巻き込まれ、8年の刑期を終えて出所した元軍人のキャメロン。彼は一刻も早く愛する妻子の顔が見たいと、連邦保安局の空輸機に乗り込んだ。だが、その空輸機は凶悪犯サイラスが率いる囚人グループにハイジャックされてしまう。

元グリーンベレーでありベトナム戦争から帰還したジョン・ランボー。戦争は終わり、たった一人生き残った戦友を訪ねた帰り道、ふらりと田舎町に立ち寄ったが、汚らしい風来坊という見た目だけで保安官にいわれの無い嫌がらせを受け、無実でありながら不当逮捕されてしまう。保安官事務所内で静かに抵抗するランボーだったが、監禁され暴行されるうち次第に戦場でかつて経験した過酷な拷問の苦痛が蘇り警官達をあっという間に打ちのめしそのまま逃亡してしまう。追われるランボーだが逃げ込む先は彼のグラウンドである山の中。ゲリラ戦で培った戦闘技術で追跡する保安官全てを圧倒的に屈服させるが、ランボーの逃走はマスコミ、州警察、軍を巻き込み次第に大事になってゆく。
