Jack Conrad is awaiting the death penalty in a corrupt Central American prison. 船舶の炎上事故を調べていた捜査官クラインは尋問していたヴァーバルから奇妙な話を聞かされる。6週間前に銃器強奪事件の容疑者として集められた5人が、釈放後、協力して宝石強奪を決行。ブツをさばくためにLAの故買屋と接触した5人は、そこで新たなヤマを依頼されるが、宝石と聞かされていた獲物は麻薬で、トラブルから相手を射殺してしまう。そして恐慌状態の彼らの前に、伝説のギャング“カイザー・ソゼ”の右腕と名乗る弁護士が現れたというのだ。

ジグソウと弟子アマンダは死んだ。ジグソウ最後のゲーム現場でパズルを解こうと奮闘するホフマン刑事。ケリー刑事殺害のニュースを聞いたFBIからストラム捜査官とペレーズ捜査官も駆けつける。しかしその頃、ジグソウに関わって唯一生き残っていたSWATのリッグが、新たなゲームに強制参加させられていた。与えられた時間は90分。すべての罠をクリアして旧友を救うことが出来るのか、それとも地獄の結末を迎えるのか。このゲームを操っているのは何者なのか？
