10대 소년 알렉스는 친구들과 극악한 비행을 저지르고 다닌다. 어느 날 차를 훔쳐 드라이브를 즐기던 이들은 작가 알렉산더의 집을 습격해 그를 폭행하고 그의 아내를 강간한다. 이튿날 알렉스는 불평을 늘어놓는 친구들을 힘으로 제압하고 자신이 무리의 리더임을 확인시킨다. 그날 밤 알렉스는 한 저택에 침입해 집주인을 죽이고 도망치던 중 친구들의 배신으로 문 앞에 쓰러진다. 검거된 알렉스는 살인죄로 14년형을 언도받는다. 2년 뒤, 알렉스는 좀 더 빨리 감옥 밖으로 나오고 싶은 마음에 재소자에게 약물과 충격요법으로 범죄에 대한 혐오를 불러일으키는 교화 방법인 루도비코 갱생 프로그램에 자원하는데...

18세기 음악의 도시 비엔나… 음악으로 신을 뛰어 넘고자 하는 욕망과는 달리 청각을 잃어가면서 자괴감에 빠져 성격은 날로 괴팍해지고 고독에서 헤어 나오지 못하는 악성 베토벤 (에드 해리스). 자신의 마지막 교향곡인 ‘9번 교향곡’의 초연을 앞두고 있던 베토벤은 자신이 그린 악보를 연주용으로 카피하기 위한 유능한 카피스트를 찾던 중 우연히 음대 우등생인 안나 홀츠(다이앤 크루거)를 추천 받는다. 단지 여성이란 이유로 카피스트 ‘안나 홀츠’ 와의 만남이 달갑지 않던 그였지만 첫 날 베토벤이 잘못 표기한 음을 간파하고, 스스로가 고쳐 그려놓은 것을 보고 그녀의 천재성을 인정하지 않을 수 없게 된다. 신의 소리를 연주하는 천재 베토벤의 음악을 가슴 깊이 이해하는 안나와 조금씩 마음을 문을 열게 되면서 이제 둘 사이에는 그 누구도 이해하지 못했던 음악적 교감뿐만 아니라, 사랑 그 이상의 영혼을 교감해 나간다. ‘9번 교향곡’ 작곡 역시 점점 더 활력을 띄며 드디어 모든 작곡이 마무리 되고, 초연의 날이 다가온다. 그러나 청력상실로 오케스트라의 연주를 들을 수 없는 베토벤이 돌연 초연의 지휘를 직접 하겠다고 나서며 뜻밖의 위기가 찾아오는데…

어느날 한 남자가 신문에서 일자리를 구하러 공사 현장을 찾아간다. 그곳에서 그는 하워드(Howard: 델로이 린도 분)를 만나게 되고 두사람의 우정은 싹트기 시작한다. 일하는 도중 그는 갑자기 환각상태에 빠져 공중묘기를 보이는 한편 비행시도를 하려다 하워드에 의해 위험을 묘면하게 된다. 그는 병원으로 옮겨졌으나 며칠 후 이상이 없다는 결과로 다시 풀려나와 은행에 근무하고 있는 여사원을 만나 하루를 보내다 연주회에서 난동을 부린 죄로 또다시 병원으로 가게 된다. 의사인 리비 박사(Dr. Libbie Bowen: 레나 올린 분)는 그의 이름이 존스(Mr. Jones: 리차드 기어 분)라는 사실을 알게 된다. 리비는 그를 치료하며 뒷조사를 해서 과거 속의 여자에 대해 알게되는데 이 사실을 알게된 존스는 그가 믿었던 리비에 대해 분노와 배신감을 느끼게 되며 병원에서 나오고 만다. 그를 따라간 리비는 존스에게 사과를 하게되고 마침내 존스와 관계를 갖게된다. 환자와 관계를 가졌다는 사실로 직업에 위협을 느낀 리비는 그사실을 동료의 사에게 고백하는데 그는 존스를 다시는 만나지 말라고하며 존스를 다른 병원으로 옮긴다. 이에 상처를 받은 존스는 얼마후 병원에서 나와 비행시도를 하러 공사가 완성된 현장으로 간다. 한편 존스가 자살시도를 할거라는 사실을 눈치챈 하워드는 리비에게 연락을 하게되며 결국 진실한 사랑의 의미를 깨달은 리비박사는 의사직을 포기하고 존스를 찾아 나선다.
