Tsukiko is part of the photography club at school, but has always felt less than her sister Tomie. She's cute and very popular among boys. However, one day, Tomie dies in a horrible accident, triggering endless nightmares for Tsukiko. One year later, right when Tomie was supposed to turn 18, someone knocks on the door. Tomie is back.

The upper torso of a female, claws her way around Japan searching for her lower half. The person's lower half was severed in a train accident in Hokkaido. Anyone that hears of this story will see Teke-Teke's lower half walking aimlessly around the countryside within three days.

The story of Teke Teke, the ghost woman with no legs, continues. Conflict arises between a group of high school classmates, and Teke Teke starts hunting them down one by one.

Some students decide to set up a play written by one of their classmates, who has commited suicide. The play is based on a tragic and mysterious legend about a love affair between two teachers at the school long ago. Some girls then accidentally stumble over some information about the legend, and the girl who commited suicide. Shortly after, the girls start turning up dead in seeming accidents.

High school student Aya Tsukimori becomes cursed and is unable to leave her school dormitory. Meanwhile, many students begin reporting ghost sightings at the school, while others go missing and are later discovered to have drowned mysteriously. Soon, Aya attempts to communicate with fellow student Michi Kazato, who begins investigating the disappearances of her friends.

Koji Shiraishi is interested in an indiscriminate attack at a sightseeing resort. He interviews the survivors and starts investigating the strange events that have been happening to them.

Nami has been creating artwork for a new video game based on images she's been seeing in her dreams. With one of the game producers, she travels out to an abandoned house that seems to match her visions. As they explore the old mansion, Nami begins to have more visions of a forgotten childhood, until at last she comes across a photo of twin infants, labelled "Nami" and "Naomi". As Nami and the producer go from room to room, an unseen person seems to be watching them from a hidden room.

O patologista Mitsuo Ando é convidado a realizar uma autópsia em seu amigo e colega Ryuji Takayama. Ao investigar as circunstâncias da morte de Takayama, Ando descobre que era um amigo de Reiko Asakawa e que ambos acreditavam na existência de uma transmissão de um vídeo que assombrada e poderia matar as pessoas, após de sete dias de assisti-lo. Ando rejeita esta ideia, mas depois de receber e assistir a uma cópia da fita ele começa a acreditar, e em seguida ter sonhos com Sadako aparecendo para ele.

No primeiro filme da série, baseada no mangá de Ito Junji, uma jovem traumatizada tenta recuperar suas memórias com a ajuda de uma psiquiatra. Durante as sessões de hipnose, ela repete o nome "Tomie", mas não consegue se lembrar de onde o conhece. Enquanto isso, um detetive da polícia está investigando uma série de assassinatos brutais, em que também o nome "Tomie" está envolvido.
