荒野を彷徨い続けるマックスは、航空機を操る謎の親子連れに乗り物や装備を奪われる。 徒歩でたどり着いた“バータータウン”は、物々交換で成り立つ街だった。街の支配者であるアウンティに腕前を買われたマックスは、自分の持ち物を取り戻すべく、全ての問題を解決するために設けられたステージ“サンダードーム”にて、街の裏の支配者であるマスター・ブラスターと戦うことになる。

Only a few people still live in New York in 2012. They are organized in gangs with their own turf. One of them is led by Baron, another one by Carrot, and they are constantly at war with each other.

症状がレベル5まで進行した感染者は、凶暴化して人間を襲い始めるという危険なウイルスが蔓延するアメリカ。ある秘密と目的を抱えながら、CDC（疾病管理予防センター）のドクターとして、感染者から未感染者を守る救助チームへの配属を希望した女性ローレン。彼女の狙いは、本来感染者は救助不能とされるところ、ドクターなど貴重な人材の場合に限り、レベル1までなら治療できる特効薬を使えるという特権にあった。

フィラデルフィアに住む元国連職員ジェリー・レインは、妻と長女レイチェルと次女コニーを学校に送るため自動車に乗っていたが、いつもとは様子の違う交通渋滞にはまってしまう。人間を狂暴化させる謎の疫病が世界各地で流行し始めており、フィラデルフィアの街はゾンビの大群に襲われていた。ジェリーは街を離れるが、携帯電話へ国連事務次長ティエリーから連絡が入り、現場への復帰を要請される。

前作から数年後、大戦が世界各地で勃発し文明は衰退し世界は貧困と荒廃が進んでいた。復讐を遂げたものの虚無感に囚われ荒野を目的もなく走り続けるマックス。ある日捕らえた男ジャイロキャプテンより今も稼動し続けガソリンが取り放題の石油精製所があると聞かされる。しかしそこにはガソリンを独占しようとするヒューマンガス率いる暴走族集団と精製所に立てこもる市民が日々攻防戦を繰り返していた。　なんとか精製所にもぐりこんだマックス。中の市民は豊富なガソリンを持って脱出をしたいがガソリンタンクを牽引する車を持って来ればガソリンを好きなだけ渡すという取引を行う。群がる凶悪暴走族の中、マックスは一人で突破を試みるが・・
