A young, depressed man is trying to move on with his life after his girlfriend's bizarre death. One day, a mysterious woman notices him at work and convinces his coworkers to introduce him to her.

A young woman named Kazue runs over a naked girl on a mountain road. When she stops to help, the girl runs off into an old abandoned house in the middle of the woods.

Within days after the mysterious new student Kawagami Tomie joins a high school classroom, all hell literally breaks loose. Simple in-fighting between boys and girls over Tomie's oppressive shadow soon gives way to mind-breaking violence, culminating in a grizzly dissection that ultimately leads to suicide, insanity, and even more violence.

Set in a house in the suburbs, the film uses black humor to depict the horrors caused by a strange drainage pipe that drags people in.

초상화를 그리고 있는 히데오의 모델이 그림을 망쳐 놓자 히데오는 팔렛 나이프로 모델을 찌른다. 피로 범벅이 된 캔버스에는 눈가에 점이 있는 여자의 모습이 그려져 있다. 그리고 이미 죽어있는 여자 앞에서 '사랑해, 토미에' 라고 중얼거리는 히데오. 다음날. 타쿠미와 준이치가 우울해 하는 히데오를 위해 미팅을 주선한다. 그런데 미팅 장소에 낯선 미소녀 한명이 나타난다. 남자들은 그녀의 미모에 반해버리지만 타쿠미와 준이치는 놀라움을 감추지 못한다. 그 소녀는 전날 밤 타쿠미와 준이치가, 히데오를 도와 숲 속에 매장을 했던 토미에였기 때문이다. 얼마 후 히데오는 화장실에서 피를 흘리며 죽은 채로 발견된다. 이어 타쿠미와 준이치, 그리고 타쿠미의 애인 히토미의 앞에 나타난 토미에. 토미에가 그들에게 나타날 때마다 기묘한 일이 생기면서, 그들을 차례로 광란과 공포의 세계로 몰고 가는데…

자폐적인 미소년 오시키리 토오루는 어릴 때부터 누군가의 그림자가 자신의 시체를 묻는 악몽을 자주 꾸어왔다. 그러던 어느 날, 친구인 미오와 함께 자신이 예전에 살던 저택에 찾아간 그에게 하나 둘 이상한 사건이 벌어진다. 저택에 숨겨진 비밀은 과연 무엇일까.

기분 나쁠 정도의 한적함에 덮혀있는 쿠로우즈 마을에 사는 소녀 키리에는 어릴 때부터의 친구였던 슈이치와 연인 사이로 발전한다. 슈이치는 최근 자신의 아버지가 이상해졌다는 말을 하며 함께 마을을 떠나자고 한다. 이후 키리에의 주변에서 이상한 일들이 벌어지기 시작한다. 한편 도예가인 키리에의 아버지에게 소용돌이 문양의 접시를 부탁한 슈이치의 아버지는 소용돌이 모양과 관련된 갖가지 물건들을 방이 가득찰 정도로 수집한 끝에, 결국 세탁기 속에서 소용돌이 모양으로 꼬여진 시체로 발견된다. 또한 슈이치의 어머니도 슈이치의 아버지를 화장하던 중 굴뚝에서 나오는 연기가 소용돌이 모양으로 바뀌는 모습을 보고 쇼크로 병원에 입원한다. 이러한 현상은 마을에 전염병처럼 퍼지기 시작하는데...

모리타 병원의 원장 모리타 겐조의 무남독녀 유미 갑자기 실종된 아버지의 행방을 찾고 있다. 아버지뿐이 아니었다. 최근 이 병원에서는 의사나 간호사들이 계속해서 실종되고 있었다. 유미는 우연히 모리타 병원에 입원해있는 대학생 사토우 후미히토와 알게된다. 둘은 사건의 수수께끼를 풀기 위해 힘을 합치게 된다. 겐조가 실종되기 며칠 전 응급환자로 밤늦게 실려온 한 소녀가 있었는데 놀랍게도 그 소녀의 몸 속에는 눈 밑에 점이 있는 살아있는 여자목이 나온 것이다. 그리고 수술에 참여한 모든 의사들은 하나같이 행방불명이 된 것이다. 병원 지하에서 목을 배양하여 관찰하고 있었던 겐조의 노트에는 미친 듯이 몇 번이고 그 여자의 이름이 씌어져 있었다. 토미에, 토미에, 토미에.... 귀신, 귀신, 귀신... 죽여라, 죽여라, 죽여라... 그 후 겐조는 유미에게로 돌아오게 된다. 그러나 돌아온 겐조는 이전의 아버지의 모습이 아니었다. 결국 겐조는 자살을 하고 유미는 더욱 더 혼란에 빠진다.
